Rachel Patoine Lepine ’01 is the Service Coordinator and Long Term Care Manager for Caledonia Home Health & Hospice, a division of Northern Counties Health Care. 

 

Rachel’s role is one that helps to ensure patients in our region receive the long term care they need to have the best quality of life possible.  Coordination of care ensures patients and their care givers play a role in their care, and that all aspects of this care lines up with their needs. The importance of this position has increased during the pandemic.

Throughout the course of the COVID-19 pandemic, Rachel has worked with the agency’s case managers to ensure the safety and well-being of some of our community’s most vulnerable population, including those residing in their homes who require nursing home level of care.  Rachel has been on the front line to coordinate the safe initiation of home health services for our community members, including those impacted by COVID-19.

 

Thank you to Rachel for working hard to ensure that Caledonia Home Health & Hospice’s services have been able to continue for those who need it the most. The compassionate care provided has been a bright light for patients during these troubling times.
